BY CHECKING THIS BOX, I HEREBY; (1) authorize Amegy Bank to open the New Account specified at the top of this application; (2) agree to be bound by the New Account Credit Card Agreement that Amegy Bank will provide to me if this Application is approved; (3) certify that all information I have provided on, or in connection with, this Application is true, correct, and complete; (4) authorize my current and past creditors, employers and consumer reporting agencies and any other reference listed above to release information to Amegy Bank regarding my eligibility for the New Account I have selected above; (5) authorize Amegy Bank to exchange information about how I handle my account(s) with Amegy Bank to credit reporting agencies if this Application is approved; (6) agree that if any wireless telephone number is provided, I consent to receiving autodialed and prerecorded message calls and text messages from Amegy Bank or its third party debt collector at any such number and represent to Amegy Bank that I have the authority to provide this consent for any wireless number provided; (7) understand  Amegy Bank will obtain periodic follow on credit reports on me from credit reporting agencies; (8) certify that I have received, read, and understand the disclosures set forth in this Application; (9) agree to immediately notify Amegy Bank Bankcard Services at 888-500-2960 (toll free) if a New Account credit card is lost or stolen; and (10) acknowledge that this Application is subject to approval by Amegy Bank.
